For Discharge Planners & Case Managers

Placing a patient in 24–48 hours? Work directly with a placement advisor.

When you're discharging a patient against the clock, you need to know which communities take the care level, accept the payer, and can actually meet the patient's needs. I know Oklahoma's assisted living, memory care, and residential care homes first-hand. Tell me the situation and I'll send back a focused shortlist that fits, at no cost to the patient or family.

Call or text. After-hours and weekends welcome for active discharges.

Turnaround:you'll typically hear back within the hour during business hours, and same day on evenings and weekends for an active discharge.

⚠ Please don't send patient details through this website.

To protect PHI, this site has no patient intake form. Share the patient's name, needs, and payer with me by phone or text, not through a web form or unsecured email. I'll take it from there.

Medicaid patient? Know your ADvantage options first.

Most Oklahoma assisted living is private-pay only. Medicaid pays for assisted living through the state's ADvantage Waiver, and only a small, specific set of communities hold that contract. These are the communities that accept it, so your team isn't working through private-pay-only buildings:

Know the level of care before you call

Oklahoma Senior Placement covers assisted living, memory care, and residential care homes. These are not skilled nursing (SNF). For a patient needing IV therapy, ventilator, complex wound care, or skilled rehab, reach out and I'll direct you to an appropriate skilled option instead.

How it works

1

Share the essentials

Care level, payer (ADvantage/Medicaid, LTC insurance, private pay), location, and special needs like memory care/secured, two-person transfer, hospice, oxygen/diabetic, or behavioral.

2

I screen to communities that fit

I match to buildings that accept the level and payer and can meet the needs, and rule out the ones that can't.

3

You receive a focused shortlist

A short list with direct introductions, so your team isn't working down a full directory.

4

Free to the family

No cost to the patient or family. One accountable point of contact through move-in.
