OSP RatingOSP ExcellentHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Excellenta consistently clean state inspection history — one of the strongest records in Oklahoma.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings

State inspectors visited Arbor House Reminisce twice in the past two years. A September 2024 complaint investigation into allegations of abuse, narcotics handling, and staffing closed with no deficiencies cited at all; the surveyor observed residents during activities, at meals, and while receiving care and medications from staff. At the October 2025 licensure visit, which also covered two complaints, the surveyor wrote that residents were clean, dressed appropriately, and free of odors, with staff assisting them with activities of daily living. That visit did cite one deficiency — the building's annual fire marshal inspection had not been done for 2025 — while the complaints about hot water and about resident elopement produced no citation. The fire marshal cleared the building on October 30, 2025 and the state verified the facility back in compliance effective that date.

  • On the October 2025 visit the surveyor toured the building and found residents clean, dressed appropriately, and free of odors. 10-15-2025
  • Staff were seen helping residents with everyday personal care during that same visit. 10-15-2025

  • Two complaints investigated in October 2025 — one about residents not having hot water, one about the building being secure enough to prevent a resident wandering off — produced no citation on either subject; the only deficiency written up was a missing annual fire marshal inspection. 10-15-2025
  • The fire marshal inspection was completed within about two weeks and the state confirmed the facility back in compliance. 10-15-2025

What the state also noted

  • The October 2025 survey was not clean: one deficiency was cited for no 2025 annual fire marshal inspection on file — an isolated finding, touching one or a few residents. OSDH's letter states the deficiency represented "the potential for more than minimal harm."
  • That the hot-water and elopement allegations were not sustained is inferred from the Statement of Deficiencies listing only the fire marshal tag; the reports do not use the word 'unsubstantiated'.
  • The September 2024 investigative report contains no observations about resident cleanliness, grooming, or odors — that language comes only from the October 2025 reports and should not be applied to the 2024 visit.
  • Compliance was verified by an offsite/paper revisit on 11/10/2025, not an on-site re-inspection.
  • The 09-03-2024 complaint number is one investigation; the October 2025 visit covered two complaint numbers (OK00075574 and OK00081257), so three complaint investigations total.
