About

Ash Street Place is an Oklahoma family-owned senior living community in the heart of downtown Guthrie's historic district—the nation's largest historic preservation district—within walking distance of shops, restaurants, and museums. The community offers comfortable, senior-friendly studio apartments (216–450 sq ft) with in-suite bathrooms and full-size showers, emergency call systems and personal pendants, individual heat and air controls, cable TV, wireless internet, and a pet-friendly policy. It carries a proud legacy of care across six decades and three generations, positioning itself as a hometown name residents can trust.

Care is organized into three levels—from independent living where residents self-administer medications, to assisted living with medication administration by trained staff and monthly RN review, up to a higher level that adds help with bathing up to three times per week, dressing, oral care, grooming, toileting assistance, and cognitive reassurance checks. All levels include planned activities, housekeeping and linen service, meal preparation, general maintenance, and 24-hour security, with restaurant-style dining available from 7 a.m. to 7 p.m. daily. Activities are tailored to residents' interests, hobbies, and culture.

OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings

State inspectors spent two days at Ash Street Place in November 2025 and cited no deficiencies at all. The previous year's inspection was not clean: surveyors found expired and unlabeled food in the kitchen and found that required monthly medication reviews by a nurse or pharmacist had been missed in six months of 2024. The facility submitted a correction plan the state accepted, and on January 7, 2025 the state conducted an offsite paper revisit and accepted that the problems had been corrected; a separate state review panel also threw out one of the 2024 citations after finding the facility's resident contracts did cover evacuation needs. The overall picture is a facility that had real kitchen and medication-oversight problems in 2024, submitted corrections the state accepted on paper, and came through its next full on-site inspection clean.

  • The most recent state inspection, a full relicensure survey in November 2025, found nothing at all to correct. 11-20-2025
  • An independent state review panel reviewed one of the 2024 citations and removed it, agreeing the facility's resident service contracts did address residents' evacuation needs. 11-17-2024
  • The facility reported the 2024 problems fixed, and on a January 7, 2025 offsite paper revisit the state accepted that it was back in compliance with licensure standards. 11-17-2024
  • The state accepted the facility's written plan to fix each problem, which included re-educating dietary staff, restarting monthly medication reviews by a registered nurse, and reporting results at quarterly quality meetings. 11-17-2024

What the state also noted

  • The November 2024 relicensure survey cited three deficiencies that were upheld: two involving food storage (expired salad dressing, tartar sauce and peanut butter; unlabeled, undated frozen and dry goods; lunch meat kept past use-by limits) and one for missing monthly registered-nurse or pharmacist medication reviews for all twelve sampled residents in February, April, June, July, August and October 2024. A fourth citation, about evacuation provisions in service plans, was removed on informal dispute resolution.
  • The state's confirmation that the 2024 deficiencies were corrected came from an offsite/paper revisit on January 7, 2025, not an on-site re-inspection, so no surveyor physically re-checked the kitchen or the medication-review records before compliance was restored.
  • The 2025 survey document is only two pages and does not describe what surveyors observed, so this bundle contains no direct surveyor observations of daily resident care.
  • No complaint investigation reports, allegations or complaint numbers appear in these records, and the survey documents describe the November 2024 visit as a relicensure survey - though one OSDH letter (the January 7, 2025 IDR determination) refers to it as 'the November 27, 2024 complaint investigation.'

Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
