Ashbrook Village had one state inspection in the past two years, conducted over two days in early May 2025. Inspectors cited one issue: for three of the eight resident files reviewed, the comprehensive assessment was not signed by the resident or their representative and did not show that the required personal interview with the resident had taken place. The administrator told the surveyor that interviews had been conducted, but the files did not document it, so the record cannot confirm either way. It was the only deficiency cited, and the state scored it at the lowest scope level. The community's plan of correction committed to including residents in their assessments and to monthly record audits, and a paper revisit on May 27, 2025 found the community back in substantial compliance.
