OSP RatingOSP ExcellentHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. What state inspectors found

2 inspections in the last two years · 2 with no deficiencies cited

Bartlesville Assisted Living has been inspected twice in the past two years, in July 2024 and July 2025, and both surveys ended with no deficiencies cited. Each visit also included a complaint investigation, and neither complaint led to a citation. In 2025 the surveyor described the building as clean with no foul odor and observed residents being treated with dignity and respect, and noted that the salon, laundry, maintenance and medication rooms were locked, exits used keypads, and the memory care area had an enclosed courtyard. In 2024, after a complaint about staffing, the surveyor counted direct care staff on duty and reviewed grievance reports and Resident Council minutes without citing the facility.

  • The July 2025 relicensure survey, which included a complaint investigation, ended with no deficiencies cited. 07-31-2025
  • The prior year's survey in July 2024, also paired with a complaint investigation, likewise ended with no deficiencies cited, giving two clean inspections in a row. 07-25-2024
  • During the 2025 visit the surveyor found the building clean with no foul odor and saw residents being treated with dignity and respect, observing no abuse of any kind. 07-31-2025
  • The surveyor noted specific safety measures: the salon, laundry, maintenance and medication rooms were all locked, exit doors used keypads, and the memory care area had an enclosed outdoor courtyard. 07-31-2025
Show all 5 findings
  • A 2024 complaint alleged there were not enough staff. The surveyor physically counted direct care nursing staff on duty, reviewed grievance reports, Resident Council minutes and staff training records, and no deficiency was cited. 07-25-2024

What the state also noted

  • The 2025 complaint involved a serious allegation of sexual abuse by staff. The state cited no deficiency and the surveyor observed no abuse of any form, but the reports do not use the word 'unsubstantiated'.
  • Survey documents are short (three to four pages each) and the narrative detail is limited compared with longer reports.
  • The documents are OCR scans with some garbled text.
