The May 2025 relicensure survey here produced two citations, and one of them is more than a paperwork problem. Two of five sampled direct-care staff files had no documentation of first aid and CPR training in a building that includes memory care — one of them a medication aide employed since November 16, 2021 — and the facility's own plan of correction states those employees were removed from the schedule pending proper certification. The second citation was a kitchen finding: six of 15 opened food items in one of the building's two freezers had no open or use-by date. OSDH classified both as representing the potential for more than minimal harm. Inspectors also investigated four complaints during the same visit, covering abuse and resident privacy, staffing and emergency evacuation, call lights, and the Alzheimer's disclosure form; no abuse, staffing, evacuation or call-light deficiency appears on the Statement of Deficiencies for that survey event, though the report does not link findings to individual complaints. The correction plan was accepted and an offsite/paper revisit on June 13, 2025 confirmed compliance effective June 6, 2025.
