Brookdale Enid's most recent full relicensure inspection, in October 2025, was clean — the state cited nothing. However, a complaint survey in April 2026 found two serious problems that caused real harm: a resident who was supposed to be transferred by two staff using a mechanical lift was moved by one aide instead and cut their leg badly enough to need the emergency room, and a resident with dementia got outside overnight after staff did not follow the door alarm policy, and was found in a dry creek bed behind the building with facial bruising and a fractured finger. Both residents were sent to the hospital. On the same visit, surveyors did see residents who were clean, dressed and free of odors, staff helping with daily care, residents in activities, and staff responding promptly when a door alarm was set off. The community retrained staff, and a follow-up state visit on June 18, 2026 confirmed all deficiencies were corrected.

What the state also noted

  • The April 10, 2026 complaint survey cited two actual-harm deficiencies, each affecting one resident: one for failing to follow a resident's service plan requiring two staff and a mechanical lift, resulting in a laceration to the leg and an emergency room visit; and one for failing to follow the door alarm response policy, resulting in an elopement in which the resident sustained facial bruising, scratches and a fractured finger.
  • In the elopement, a CNA found the east door cracked open with the alarm sounding between 1:30 and 2:00 a.m., called out and looked around, then turned the alarm off and shut the door without locating the resident. The resident with vascular dementia was not found until the morning bed check, in a creek bed behind the facility, and was sent to the emergency room.
