Brookdale Stillwater's March 2025 inspection was unusually thorough: the state combined its regular relicensure survey with four separate complaint investigations covering care delivered per physicians' orders, staffing and call light response, food quality, family notification, cleanliness and pest control, missing personal property, and the accuracy of medication and controlled-substance records.
