Brookdale Stillwater's March 2025 inspection was unusually thorough: the state combined its regular relicensure survey with four separate complaint investigations covering care delivered per physicians' orders, staffing and call light response, food quality, family notification, cleanliness and pest control, missing personal property, and the accuracy of medication and controlled-substance records. Surveyors toured the building, watched staff with residents, and reviewed health records, grievances, police reports and cleaning and pest control logs. Most of those allegations led to no citation — but one did not clear. The state cited the community, recording the problem as a pattern affecting several residents, after finding that a dose of alprazolam, a controlled anti-anxiety medication, was administered to a resident on 03/13/25 but was never signed out on that resident's individual narcotic log — the same failure one of the complaints had alleged. The facility's health and wellness director described it as a documentation error; the facility re-in-serviced medication staff and added weekly narcotic count audits, and a state revisit in May 2025 confirmed it was back in substantial compliance effective May 8, 2025.
