OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

1 inspection in the last two years · 1 with findings

Brookdale Stillwater's March 2025 inspection was unusually thorough: the state combined its regular relicensure survey with four separate complaint investigations covering care delivered per physicians' orders, staffing and call light response, food quality, family notification, cleanliness and pest control, missing personal property, and the accuracy of medication and controlled-substance records. Surveyors toured the building, watched staff with residents, and reviewed health records, grievances, police reports and cleaning and pest control logs. Most of those allegations led to no citation — but one did not clear. The state cited the community, recording the problem as a pattern affecting several residents, after finding that a dose of alprazolam, a controlled anti-anxiety medication, was administered to a resident on 03/13/25 but was never signed out on that resident's individual narcotic log — the same failure one of the complaints had alleged. The facility's health and wellness director described it as a documentation error; the facility re-in-serviced medication staff and added weekly narcotic count audits, and a state revisit in May 2025 confirmed it was back in substantial compliance effective May 8, 2025.

  • One of the four complaints investigated in March 2025 raised four separate concerns — that care was not provided according to physicians' orders or the care plan, that there were not enough staff and call lights were not answered in a timely manner, that the food served was not palatable, and that residents' representatives were not notified of a change in condition — and none of those four allegations resulted in a citation. 03-19-2025
  • A complaint alleging the building was not clean, comfortable and homelike and lacked effective pest control was investigated, including a review of the facility's cleaning and pest control records, and no environmental deficiency was cited. 03-19-2025
  • A complaint that residents' personal property had been taken was investigated — surveyors reviewed police reports and asked residents and staff about personal items and funds — and it did not result in a citation. 03-19-2025
  • The surveyor toured the building and observed residents specifically for staffing, dietary, and abuse and neglect concerns, and watched staff interacting with residents. 03-19-2025
Show all 5 findings
  • The single deficiency the state cited was corrected, and a state revisit in May 2025 found the community back in substantial compliance. 03-19-2025

What the state also noted

  • One deficiency was cited under the medication record rule, at a level indicating a pattern affecting several residents, for failing to maintain an accurate individual inventory record for a schedule IV controlled medication — the dose was administered but not signed out on the individual narcotic log.
  • That citation matches allegation 2 of complaint #OK00079245, which alleged the center failed to maintain an accurate written record for medications and scheduled controlled medications — so one of the four complaint allegations was borne out by the survey, not dismissed.
  • The medication review sampled only 5 of the 22 residents receiving medications, so the survey does not establish whether other narcotic log entries were accurate.
  • Only one survey falls within the last 24 months, so there is limited recent history to judge consistency by.
  • The survey contains no explicit surveyor statements that residents were clean, well-groomed, or free of odors; the positives above rest on allegations investigated without citation, not on affirmative findings of good care.

Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
