State inspectors were at Brookridge twice in the past two years — January 2025 and November 2025 — and both visits produced citations, so this is not a clean record. The January 2025 visit was a complaint investigation into pests, and both complaints were substantiated: a live cockroach was observed crawling under the kitchen prep area, a dead one outside the kitchen bathroom door, dead roaches on catch strips, a staff member preparing the lunch meal without a hair net, a trash can with thick dried food particles, dried food on the serving-line shield, six undated pies in the refrigerator, and an uncovered garbage can beside the food prep area. The facility's last pest-control log entry was dated 07/26/24 and maintenance kept no documentation of what was used or whether it worked. OSDH cited kitchen sanitation and failure to maintain an effective pest control program. The November 2025 relicensure survey included a complaint that medical information was not kept confidential; that allegation was not upheld — surveyors found records and computer screens locked and confidential information stored in designated private areas, and observed staff answering call lights, giving medications, serving meals and assisting residents. That survey still produced three citations: a scoop stored inside flour and bread-crumb bins, an activities staff member hired in March 2024 who had never received the required activities training, and one of five sampled direct-care staff files with no proof of first aid training; HR told the surveyor no staff training policies existed. OSDH classified the deficiencies from both surveys as representing potential for more than minimal harm, with no actual harm identified. The facility corrected both sets and the state verified substantial compliance by offsite/paper revisit on 02/14/2025 and 12/05/2025.
