OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

What state inspectors found

1 inspection in the last two years · 1 with findings

Canoe Brook Ardmore's one inspection in the last two years, in November 2025, was a demanding one: the state arrived unannounced with five separate complaints to investigate, including allegations of abuse, too little staff, poor meals, and medication problems. After three days of observing care, reviewing records and interviewing residents and staff, inspectors did not cite any of those allegations. The only deficiency written up was open containers of sugar, dry beans and croutons left uncovered in the kitchen dry-storage area, which could attract pests. The community corrected that and a December 31, 2025 state review found it in substantial compliance.

  • Inspectors arrived unannounced with five separate complaints to investigate at once, covering abuse, staffing levels, meal quality and therapeutic diets, medication administration, and building equipment. After three days on site, the only thing they wrote up was open food containers in the kitchen storeroom. 11-05-2025
  • The most serious allegation, that residents were not free from physical, verbal, and psychosocial abuse, was investigated in depth. The surveyor watched how staff treated residents, reviewed incident reports and abuse training records, and interviewed residents and staff about identifying and reporting abuse. No abuse-related deficiency was cited. 11-05-2025
  • Surveyors watched residents throughout the building, in their rooms, in common areas, at meals, during medication passes and in activities, and saw staff interacting with residents and responding to their needs. 11-05-2025
  • Complaints about not having enough staff and about food being unpalatable or not meeting special diet needs were checked against staffing schedules, menus, weights, diet orders and resident interviews. No staffing, dietary-order or nutrition deficiency was cited. 11-05-2025

  • A complaint alleging medication errors and a failure to assess and act on residents' changing conditions was investigated, including a review of medication administration records and direct observation of a medication pass. No medication deficiency was cited. 11-05-2025
  • The November 3 through 5, 2025 relicensure survey, conducted alongside five complaint investigations, did result in a citation: open dry-food containers in the kitchen storage area, which the state recorded as a pattern affecting several residents. One section of the state's form was closed out with no deficiencies cited; another section states that deficiencies were cited. The citation was accepted for correction and verified corrected at an off-site revisit on December 31, 2025. 11-05-2025
  • The kitchen storage issue was corrected. The community put dry-food storage logs and daily manager checks in place, and a December 31, 2025 state review found it in substantial compliance. 11-05-2025

What the state also noted

  • One deficiency was cited on 11/05/2025: food not stored in a manner to prevent conditions that could attract pests, observed over three consecutive days in the dry-storage area.
  • That citation carried a scope/severity of E, meaning the state considered it a pattern rather than an isolated slip, and the state's letter described the deficiency as representing 'the potential for more than minimal harm.'
  • OSDH investigative reports do not formally state whether an allegation was substantiated; the conclusion that the five complaints did not result in citations is based on the deficiency list on the state form, which contains only the food-storage tag.
  • Only one survey falls within the 24-month window, so there is no earlier inspection here to compare against.
