Carriage House Homes is a five-resident home, and its one inspection in this window, on April 30, 2025, cited two deficiencies; the staffing one is the reason to read carefully. Three of the five aides providing hands-on care did not hold current Long Term Care Aide certification: one aide's certification had expired in November 2024 and the administrator confirmed that aide had worked the night before the survey, and two others — hired in November 2021 and December 2022 — had never been certified at all, one having trained during COVID but never taken the test. Asked when the first aide's certification expired, the administrator told the surveyor, "I don't know off the top of my head. It's probably expired." The home's internal quality assurance committee, required to meet quarterly, had no documented meetings from January 2024 through April 2025; the administrator said, "We are supposed to have the meetings, but we don't," and that it had been over a year since the last one. A separate complaint that the home was dirty, had offensive odors and pet waste was not substantiated — the surveyor arrived unannounced at 7:40 a.m. and found the inside clean, odor-free and uncluttered with no pet waste inside or out, and residents, family and staff confirmed it — and the state accepted the home's correction plan, with an offsite paper revisit on July 23, 2025 finding the home back in substantial compliance effective June 30, 2025.

A complaint claimed the home was dirty, smelled bad, and had pet waste. The surveyor arrived unannounced early in the morning and found none of it - inside the home was odor-free, surfaces were clean, and there was no clutter. 04-30-2025

What the state also noted

— The July 2025 follow-up was an offsite paper revisit rather than an on-site re-inspection.
— The survey's initial-comments block lists survey dates of 01/01/25 through 01/05/25 while the rest of the document shows an exit date of 04/30/25 - an apparent typo in the state form.
— The certification problem was not brief or clerical. Two of the three aides had never held Long Term Care Aide certification at all, across tenures that began 11/13/21 and 12/03/22; the third's certification had been expired since November 2024; and the administrator did not know their status when asked. The quality assurance committee that is required to meet quarterly had no documented meeting from 01/09/24 through 04/20/25, so the home's own internal check had not run during that period.
— Two deficiencies were cited, one under the minimum certified staff rule and one under the quality assurance committee rule, so this was not a clean survey.
— The staffing citation is significant: the administrator confirmed three aides providing direct care were not currently certified, one having worked the night before the survey.
— Only one survey in the last 24 months is included, so there is limited history to judge trends.
