Built in 1999, Chisholm Trail Assisted Living is the only family-owned and operated assisted living center in Duncan, Oklahoma. The community emphasizes a home-like environment with individualized care plans and prioritizes compassion, dignity, and quality of life while helping residents maintain their independence. Leadership includes owner/administrator Dustin Cox and Medical Director Dr. Demetra Cox, both of whom are described as passionate about senior care.
Daily life includes three restaurant-style meals a day, nursing care with medication management and care coordination, and assistance with daily living such as meal preparation, hygiene, cleaning, and supervision, provided by licensed caregivers around the clock. When the state visited Chisholm Trail in October 2025, it was investigating three complaints at once - about abuse, medications not being given as ordered, and pests. Over three days surveyors watched meals, medication passes, activities and hands-on care, interviewed residents and staff about abuse and medications, and reviewed grievance logs, resident council minutes and pest control invoices; none of those allegations resulted in a citation. Two deficiencies were cited, both involving resident assessments - the documents that set out what care a resident needs. One resident's admission assessment was never signed or coordinated by a nurse or physician; a second resident, admitted with a fractured right femur, osteoporosis and a history of falls, had no admission assessment that staff could locate at all; and yearly reassessments were late or never completed for two of ten residents reviewed. The facility retrained its nursing leadership and reviewed resident charts, and the state confirmed in writing on December 5, 2025 that the deficiencies had been corrected effective December 1, 2025.
