What state inspectors found

This community's October 2024 relicensure inspection came back with no deficiencies cited. When inspectors returned in November 2025 to investigate three complaints about falls, supervision, and abuse, they saw staff helping residents with daily tasks, mobility, and transfers, heard residents say staff checked on them often and that they had alert wristbands to call for help, and heard families say the staff provided good care. The record is not spotless: the state cited the facility for not reporting two resident-to-resident abuse allegations to the health department within the required one business day - one of those incidents involved a resident being pushed to the floor and suffering a hip fracture. The facility retrained staff, put reporting guidelines in place with weekly audits, and the state confirmed compliance in January 2026; it is worth asking the administrator how those audits have gone since.
