Fair — a solid, generally acceptable record, with a few state findings worth a quick question.
📞Often nothing to worry about. The state’s findings are summarized below — call Paul if you want help judging whether any of them would matter for your loved one.
Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.
2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings
Covenant Living at Inverness has one serious finding and one clean record in the past two years, and families should know both. In September 2024 the state substantiated physical abuse after an aide slapped a resident with Alzheimer's on the hand four times and grabbed a second resident's forearm hard enough to make them cry out; the community itself reported it to the state, called the police and Adult Protective Services, and fired the aide within days. Since then the record has been clean: the April 2025 licensing inspection, which included a separate complaint investigation about medication access, closed with no deficiencies cited at all. Two staff the surveyor interviewed in 2024 could explain their abuse training and said they would report abuse immediately. Beyond that, these reports contain no surveyor findings about day-to-day care — no observations of hygiene, meals, activities or staffing levels — so families should ask the community directly about those, and about what supervision changed after September 2024.
Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.

Call me before you call them. Once you contact the community directly, they treat you as a walk-in — and I usually can't step in as your advocate anymore. Reach me first and you keep someone on your side for the whole search, free. There are a couple of things about this community I'd tell you on the phone that aren't on this page.
