What state inspectors found

Duncan Assisted Living was inspected three times in the last two years, and two of those inspections ended with no deficiencies cited at all — including the most recent one in March 2026. Across six complaint investigations covering abuse, supervision, staffing, pest control, and medication security, surveyors observed residents at meals and activities, watched staff assist residents in what they described as a dignified and respectful manner, and found medication carts properly locked. The November 2025 licensure survey did cite one problem: for one of five employee files reviewed, the criminal background check and offender registry check were not done until nearly five months after the employee's hire date. The facility corrected that, and the state verified in January 2026 that it was back in compliance.
