Eden Care - Oak Tree

13701 SOUTH SANTA FE, EDMOND, OK 73025

About

Eden Care Homes provides 24-hour assistance with the activities of daily living for up to five older adults in a traditional single-family home, and the Oak Tree home is located in Edmond, Oklahoma. Rather than an institutional facility, the model offers a natural, residential home setting where residents receive close, personalized attention. The operator has been serving Oklahomans for more than 14 years.

The small-home approach allows care to be adjusted to each resident's individual needs and gives families peace of mind, as reflected in resident-family testimonials describing a comfortable, at-ease, home-like environment. Because each home serves only up to five residents, caregivers can provide a high ratio of hands-on daily-living support in a quiet, family-style atmosphere.

OSP RatingOSP ExcellentHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Excellenta consistently clean state inspection history — one of the strongest records in Oklahoma.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings

Eden Care - Oak Tree is a small five-bed home in Edmond. Its most recent state inspection, in November 2025, found no deficiencies at all, and a complaint filed about its license renewal was investigated on site and not sustained — the surveyor saw a current license posted, watched staff giving care and medications, and interviewed residents about their safety and satisfaction. The record is not spotless: the November 2024 inspection did cite two problems, that residents could not control the temperature in their own rooms and that breakfast and dinner menus had not been reviewed by a dietitian. Both were corrected and the state confirmed compliance in January 2025.

  • The most recent state inspection, in November 2025, ended with a clean report — the state found nothing wrong. 11-12-2025
  • A complaint claiming the home had not renewed its license on time was investigated on site, and the surveyor found a current license posted on the wall when they walked in. No deficiency came of it. 11-12-2025
  • During that visit the surveyor watched staff caring for residents and giving medications, and saw residents spending time with staff and with one another. 11-12-2025
  • Inspectors went through medical records, physician orders, medication records, care plans, incident reports, employee files and training records, and talked with residents themselves about whether they felt safe and were happy with their care. 11-12-2025

  • The two problems found in the November 2024 inspection — residents not being able to control the temperature in their own rooms, and breakfast and dinner menus that had not been approved by a dietitian — were fixed, and the state verified the correction in January 2025. 11-25-2024

What the state also noted

  • The November 2024 relicensure survey cited two deficiencies (resident temperature control and dietitian-approved menus); the facility's first plan of correction was rejected as inadequate before an amended one was accepted.
  • This is a five-bed home, so inspection samples are very small (five residents) and a single finding affects everyone reviewed.
  • Only two surveys fall within the last 24 months.

Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
