Epworth Villa is a nonprofit, faith-based Life Plan community in Oklahoma City, affiliated with the United Methodist Church, offering independent living, assisted living, memory support, and skilled nursing on a single campus. Its assisted living provides personalized support designed to maintain residents' independence while addressing individual needs, from gentle reminders to comprehensive help with daily activities.

The community maintains 24-hour nursing availability and on-site physicians, integrating medical oversight into daily life, and partners with an on-site OU Health geriatric medicine clinic. What state inspectors found

Of this community's two state inspections in the past two years, one closed with no deficiencies cited at all, and in the other the state confirmed several of the complaints it was investigating were not borne out. Inspectors saw staff answering call lights and helping residents with daily care, watched medications being given on time as ordered, and found staff present in the dining rooms on every floor. The February 2025 survey did cite two medication-safety problems: nebulizer equipment that was not cleaned and stored per policy, and an unlocked medication cart plus medication left on top of a treatment cart. The community corrected both, and the state verified compliance in May 2025.

  • The June complaint alleged the center failed to keep a resident from eloping. Inspectors reviewed clinical records, care plans, incident reports and facility policies, interviewed resident representatives and staff about supervision and elopement, and cited no violation. 06-05-2025

  • During the February 2025 investigation, two of the ten residents interviewed said medication aides left medications with them to take later. Inspectors then observed staff across various shifts and watched medication administration, and found no evidence of medications being left unattended with residents or in resident rooms. No citation was issued on that point. 02-27-2025
  • The same complaint alleged medications were given late. Record review and direct observation showed medications were given on time and as the doctor ordered. 02-27-2025
  • Complaints about kitchen sanitation and unsupervised dining rooms were also not borne out; inspectors found sanitary kitchen practices and staff present in the dining areas on every floor during meals. 02-27-2025
  • The two problems found in February 2025 were fixed, and the state confirmed on an offsite paper revisit in May 2025 that the community was back in substantial compliance. 02-27-2025

What the state also noted

  • Only two surveys from the last 24 months were available for review, and the June 2025 record is brief (three pages).
  • The May 2025 compliance verification was an offsite/paper revisit based on the submitted plan of correction, not an on-site re-inspection.
  • The February 2025 survey cited two deficiencies, both medication-related: nebulizer equipment not rinsed, sanitized and stored per the center's policy, and an unattended unlocked medication cart with medication left on top of a treatment cart in the memory care hallway.
  • Separately from the four allegations, two of the ten residents interviewed in February 2025 told surveyors that medication aides left medications with them to take later. The surveyor's own observations found no evidence of this practice, but the residents' statements stand in the record and were not otherwise resolved.
  • One of the four February complaint allegations - unlocked carts and medications stored on top of carts - was confirmed by the inspector and resulted in a citation. The medication-cart citation was scoped at the pattern level, meaning it involved several residents rather than an isolated instance.
  • OSDH survey documents do not formally label complaints 'substantiated'; the substantiated count here reflects whether a citation was issued on the same subject as the allegation.
