What state inspectors found

1 inspection in the last two years · 1 with no deficiencies cited

Oklahoma health inspectors visited Featherstone Assisted Living Community of Durant in March 2025 for a full relicensure survey and also looked into three separate complaints at the same time. They came away citing no deficiencies. Surveyors specifically checked call light response logs, watched meals being served, and toured the building, finding calls answered in a reasonable time, adequate portions with an alternate menu offered, and clean hallways and homelike rooms with residents' own furniture and photos. The complaints about admitting residents needing too much care, hygiene and call lights, medication timing, and meals did not result in any citation.

  • The state's March 2025 relicensure inspection, which was combined with three separate complaint investigations, ended with no deficiencies cited at all. 03-13-2025
  • A complaint claimed call lights were not answered promptly. The surveyor checked the facility's call light logs and found calls were answered within a reasonable time. 03-13-2025
  • A complaint alleged meal portions were too small and menus were not followed. The surveyor observed meals directly and found portions adequate, with an alternate menu available. 03-13-2025
  • A complaint about cleanliness was investigated by touring the building several times. The surveyor found clean hallways with no foul odor. 03-13-2025

  • Resident rooms were described by the surveyor as clean and homelike, with residents' own recliners, wall pictures, and photos at their bedsides. 03-13-2025
  • Two complaints questioned whether the facility admitted residents needing more care than it could provide. Surveyors reviewed resident assessments and found the facility able to provide the care described in its contract and handbook. 03-13-2025

What the state also noted

  • Only one survey (March 2025) falls within the 24-month window, so this reflects a single point in time.
  • Complaints alleging delayed incontinent care, missed showers, delayed linen changes, and late medications were part of the investigation, but the reports do not describe what the surveyor observed on those specific points beyond the overall finding of no deficiencies.
  • The documents are OCR scans of paper forms and contain some garbled text.
