State inspectors were at Featherstone on three days in November 2025 (Nov. 3, 5 and 6) for the annual relicensure survey and to investigate two complaints. Several of the most serious allegations were not borne out: the administrator was on site when inspectors arrived unannounced, residents were seen coming and going from the building, and nothing was cited about room temperatures, hazards, supervision, or residents' choice of physician, hospice or home health. Inspectors did cite seven deficiencies: kitchen problems (no hand-washing or glove changes during meal plating, no hair net, unlabeled and undated food in the refrigerator, and dishwasher sanitizer checks not done since July 20), no 2025 fire marshal inspection on file, no licensed dietician or nutritionist reviewing residents' diets and weights, an assessment and a service plan that did not reflect one resident's oxygen use, that same resident's oxygen order not being transcribed when they returned from a skilled nursing facility, and two staff files missing proof of abuse training. The assessment, service-plan and medication findings all involved the same resident — one of the eight whose records were reviewed — who has congestive heart failure and was observed wearing oxygen at 3 liters with no active order in the chart. Inspectors found no weight loss or weight gain concerns among residents. Featherstone's plan of correction was rejected by the state twice, and the records available here do not show it was ever accepted or verified.
