Forest Hills had one state survey in the last two years, in February 2025, and it came with two complaint investigations - both alleging staff had not properly assessed and treated a resident in severe pain. Inspectors toured the building, observed residents and staff, and reviewed health records, medication records, incident reports and grievances, and no deficiency was written about pain management or resident care. They did cite the kitchen: twelve opened food items in the refrigerator and freezer were not labeled with open and use-by dates. The center relabeled everything the same day, retrained kitchen staff, and a state follow-up in March 2025 confirmed it was back in compliance.
