Fountainbrook's most recent state visit, in June 2025, was a licensing inspection combined with three complaint investigations, and inspectors cited seven problems - among them bathroom water hotter than the 115-degree safety limit in three of four rooms tested, a missed reassessment after a resident's health changed, an apartment bathroom left in disrepair for months, a wound with no record that it was assessed or treated, and an abuse-allegation incident report that did not reach the state until two days after the incident, past the one-business-day deadline. On the more reassuring side, an earlier complaint investigation in October 2024 closed with no citations at all, and when the state investigated a June 2025 complaint about pests - touring the building, reviewing pest control records, and interviewing residents and staff - none of the problems it wrote up involved pests. The community submitted a correction plan the state found acceptable, and reported that it cancelled the move-out notice for the resident involved, who continued to live there. If you are considering this community, it is fair to ask the administrator directly what has changed since June 2025 and whether the state's follow-up visit confirmed the fixes.
