Glenhaven's only state inspection in this window — a relicensure survey concluded October 29, 2025 — was not a clean one. Inspectors cited the center for allowing staff to work with residents before their criminal background checks were completed, in two of five employee files reviewed: a medication aide hired April 22, 2025 whose check was not completed until October 28, 2025, the day the surveyor asked for the records, and a nurse aide hired in November 2024 with a check dated December 27, 2024. OSDH classified the deficiency as carrying the potential for more than minimal harm, and the facility's own plan of correction accepts that all 25 residents had the potential to be affected; background screening is a step a family has no way to verify for itself. The remaining three files were in order, the director of nursing told the surveyor an onboarding system is now in place where the previous administrator had none, and the facility states all employees were re-verified on November 14, 2025 with monthly audits — a November 24, 2025 revisit recorded the center back in substantial compliance, though it was an offsite paper review rather than a return visit. This is the only survey on file for the period and it contains no surveyor observations of daily resident care, so there is limited history here either way.
