What it’s like

Go Ye Village is described as eastern Oklahoma's largest not-for-profit provider of senior housing and services, a faith-based ministry operating in Tahlequah for more than 45 years under the mission 'Serving God and Serving You.' The campus offers a full continuum of lifestyle options — independent living, patio and garden homes, an Enhanced Life tier, assisted living and a health center — in a homelike, family-centered environment with on-campus amenities including a chapel, wellness center, learning center and dining.

In assisted living, a nurse is available 24/7 to provide a helping hand and round-the-clock support for residents who need assistance with daily activities or medication management, with individual care plans designed for each person. Personal care includes medication management and help with bathing and dressing, and daily life is supported by three chef-prepared meals a day, weekly housekeeping, laundry service, scheduled transportation for shopping, salon services and planned activities and social gatherings.

State Inspection Record

Oklahoma inspects every assisted living and memory care community on a regular schedule, plus any time a complaint comes in. Inspectors are state employees, not part of this site — what follows is what they found, in their own words.

Built by Paul Swales, a senior placement advisor, from Oklahoma State Department of Health inspection records — not from reviews. Assisted living and memory care have no federal star rating.

What state inspectors found

1 inspection in the last two years · 1 with findings

When state inspectors spent three days at Go Ye Village in late June and early July 2025, they wrote that residents were treated with respect and dignity and that staff were seen throughout the visit interacting with residents and redirecting them as necessary. The visit included a complaint investigation into whether the community supervised residents well enough to prevent someone wandering off; inspectors checked all entry and exit doors, reviewed resident charts, incident reports and staff training, and interviewed residents, families and employees, and no supervision or elopement violation was cited. Two problems were cited: opened food in one refrigerator and one freezer that lacked open and use-by date labels, and an annual fire inspection that had not been done in 2025 — the most recent one on file was from March 2024. Both were corrected on paper, and the state confirmed on September 8, 2025 that the community was back in substantial compliance effective August 20, 2025.

  • Over all three days of the inspection, the surveyor saw staff interacting with residents and redirecting them as necessary. 07-01-2025
  • The surveyor wrote plainly that residents were treated with respect and dignity. 07-01-2025
  • A complaint claimed the facility did not supervise residents well enough to prevent wandering off. Inspectors checked every entry and exit door, reviewed resident charts, incident reports and staff training, and interviewed residents, families and employees. No supervision or elopement violation was cited. 07-01-2025
  • The two problems that were found were fixed, and on a September 2025 follow-up the state confirmed the community was back in compliance. 07-01-2025

  • The state accepted the community's written correction plan, which included a new food labeling and dating policy, staff training, and regular refrigerator and freezer audits. 07-01-2025

What the state also noted

  • Deficiencies were cited in this survey — it was not a clean inspection. Two violations were issued: unlabeled opened food items, and a missed 2025 annual fire inspection (the last one on file was March 2024).
  • On the fire inspection, the accepted correction plan only shows a fire marshal visit scheduled for August 20, 2025, with the report to be 'forwarded as soon as received.' Nothing in this file confirms the 2025 inspection was completed and passed. Families may want to ask to see the current fire inspection certificate.
  • Only one survey from the last 24 months is in this file, so there is limited history to judge consistency over time.
  • The elopement complaint is reported as investigated with no related citation, but the report does not use the word 'unsubstantiated.'
  • The survey does not discuss medication administration, staffing levels, or call-light response, so nothing can be said about those areas either way.
  • Correction was verified through an off-site paper revisit, not a new on-site inspection.

Full inspection history

Every inspection Oklahoma has on record for this community. The rating is based on the last 2 years, so a community is not scored on something it corrected long ago — but the older record is here to read either way.

My help finding the right community costs your family nothing.

You never pay me — the community pays my fee, and only if someone I introduce moves in. And one person calls you back: me, not a call center.

  • The price above is not the price you’ll pay. Care-level charges, community fees, medication management and second-person fees all stack on top of that base rate. I’ll walk you through what this community really charges for someone in your loved one’s situation — and push to get the move-in fee and deposit reduced.
  • Whether they have a room, and would take your loved one. I’ll find out what’s actually open right now and whether they’d genuinely accept this level of care, before you spend a morning touring.
  • How it compares to the others nearby. I hold the same state record for every community in the metro. Their sales office will never tell you the place four miles away has a cleaner history.

Paul Swales, Senior Placement Advisor at Oklahoma Senior Placement

I’m Paul Swales, and this is what I do all day. A senior placement advisor is the person who finds the right community for your family and does the hard part for you. I’ve read the state health department’s inspection record on every community in this metro. I know which ones will genuinely take your loved one’s level of care, and I make the calls — so you’re not doing this alone at eleven at night on your phone.

It works like a buyer’s agent in real estate. Your family never pays me. The community pays my fee, and only if someone I introduce actually moves in. There’s no cost to your family and no obligation to keep using me.

The one catch is the order. They only pay my fee if I introduce you before you contact them yourself. Call them first and you’re a walk-in — I can’t step in as your advocate after that. It costs your family nothing either way. It only decides whether you have someone on your side.
