Grandwood's full state relicensure inspection in November 2025 was clean - no deficiencies were cited at all. A separate complaint survey in February 2026 looked at two allegations: one about supervision to prevent residents wandering off, which produced no citation, and one about staff not assessing residents in a timely way, which the state did sustain. In that case, an aide radioed a nurse that a hospice resident was having difficulty breathing; the nurse said she would notify hospice but left the building about 15 minutes later without assessing the resident, and hospice had no record of her call. A medication aide called hospice after learning the resident had not been assessed. In its plan of correction, the center says it suspended and terminated that nurse, educated staff on February 6, 2026 and created a new emergency policy on February 13, 2026 - those steps are the center's own account, not surveyor findings. The state accepted the plan, and an off-site paper revisit on February 26, 2026 confirmed the center was back in substantial compliance.
