Green Tree at Sand Springs had one state inspection in the past two years, in April 2025, which combined a routine licensure survey with a complaint investigation of a building housing 79 residents. The complaint alleged the building was dirty and had odors, that medications were not given as ordered, and that families were not told about changes in condition. Over two days the surveyor toured the facility and reported seeing staff assisting residents and apartments being cleaned with no odor detected anywhere, reviewed health records and physician orders, and interviewed residents, families and staff, and none of those allegations resulted in a citation. The one deficiency cited was in the kitchen, where opened food in the refrigerator, freezer and dry storage was not labeled with open and use-by dates; the facility corrected it and a June 5, 2025 revisit found it back in substantial compliance.
