Greenbrier Village had one state inspection in the last two years - an October 2025 relicensure survey combined with a complaint investigation. The surveyor spent time watching residents in their rooms, in common areas, at meals and during activities, and noted staff assisting residents with their needs and interacting with them. Out of a broad review of eight residents' records, policies and grievances, the state cited one deficiency: one resident missed three scheduled doses of prescribed eye drops in February 2025 and the medication record was left blank. The facility retrained staff on medication administration, added daily and weekly checks of the medication records, and the state confirmed in December 2025 that the facility was back in compliance.
