OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

What state inspectors found

1 inspection in the last two years · 1 with no deficiencies cited

When Oklahoma inspectors visited Homestead of Bethany unannounced in March 2025, they were following up on a complaint that residents weren't receiving care according to their doctors' orders and care plans. After touring the building, reviewing charts and incident reports, and interviewing residents, staff, and family members, the surveyor wrote that no evidence was found to support the allegation. Every resident observed was well groomed, free of odors, and showed no signs of distress, and medication carts and housekeeping doors were properly locked. No deficiencies were cited.

  • The March 2025 relicensure inspection, which included a complaint investigation, resulted in no deficiencies. 03-19-2025
  • A complaint alleged residents weren't getting care according to doctors' orders and their plan of care. The state investigated and found nothing to back it up. 03-19-2025
  • On an unannounced morning tour, the inspector found five staff on duty, no odors, and medication carts and housekeeping doors properly locked. 03-19-2025

What the state also noted

  • Only one survey falls within the last 24 months, so this reflects a single inspection rather than a pattern over time.
  • The facility's site record lists two surveys in three years; only the one falling within the last 24 months was reviewed here, so an earlier survey is not reflected in this record.
  • The report notes five employees on duty at the time of the tour but does not evaluate whether staffing levels are adequate overall, so this should not be read as a finding about staffing.
  • The surveyor noted medication carts and housekeeping doors were locked; the report does not describe what was stored behind them.

Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
