In October 2024 state inspectors investigated two complaints at Homestead of Clinton, looking into concerns about abuse and neglect, staffing, medications, call lights, and help with daily activities. They cited the center on one item, and it was not a minor one: for two of the three residents whose falls were reviewed, the center "failed to follow standards of care and to implement interventions after a fall." Each of those two residents - both with dementia - had fallen three times during 2024, and the director of nursing confirmed no interventions had been added to their service plans and that facility policy was not followed. A family member told the surveyor the resident "keeps falling and the center does not seem to be able to prevent the falls." Inspectors identified no actual harm, and none of the other allegations they investigated led to a citation. The center submitted an accepted plan of correction, and on an offsite/paper revisit (no return visit to the building) the state determined on November 26, 2024 that the center was back in substantial compliance.
