2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings
Homestead of Del City's most recent state inspection, in November 2025, found no deficiencies at all. That is a real improvement over the September 2024 survey, which did cite problems - most seriously, the state substantiated that a resident was not kept free from abuse after an incident between two memory care residents, and also cited a resident's room not receiving the trash and housekeeping service that resident's service plan called for, and a plan of accommodation missing a required physician signature. The records show staff intervened immediately when the resident called for help, the facility submitted a correction plan, and the state confirmed in October 2024 - through an offsite paper review of the facility's documentation - that all cited problems had been corrected. If this facility is on your list, it is fair to ask the administrator directly about the 2024 abuse citation and what has changed since.
Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
