Oklahoma health inspectors spent three days at Homestead of Elk City in June 2025, combining a full relicensure inspection with a complaint investigation, and cited no deficiencies. The complaint alleged that staff had not recognized or followed a resident's code status; the surveyor investigated on site, observed staff with residents, reviewed health records, incident reports and grievances, and questioned residents and staff about resident rights, CPR and abuse training, and no citation resulted. This is the only survey on file for the past 24 months, so it reflects one point in time rather than a long track record.
