Inspirations of Bartlesville has one state survey on record in the last two years, and it was not clean. The November 12-13, 2025 licensure survey cited the facility because, for 2 of 5 sampled employee files, it could not show that the required criminal history background checks and offender registry checks had been obtained; both employees were certified nurse aides hired in July and August 2025 in roles with direct resident contact, and the facility's own correction plan states those individuals were still "in the process to be completed by 12/15/2025" — meaning two aides with direct resident access worked roughly three to four months without verified criminal-history clearance. This is a resident-safety requirement, not a clerical technicality, and the state recorded it as a pattern affecting several residents. The administrator told the surveyor that the two had worked for the facility's previous owners and that their company had only recently purchased the facility; the facility then audited all employee files, added a new-hire checklist, and committed to monthly new-hire file audits for six months. The state accepted that plan and, in an off-site paper revisit on December 22, 2025, found the facility in substantial compliance effective December 15, 2025; the report contains no surveyor findings about the care residents were receiving.
