Iris Memory Care of Edmond is a residential memory care community at 2424 NW 178th St in Edmond, dedicated exclusively to Alzheimer's and dementia care as a home-like alternative to traditional assisted living. The community is built as two separate buildings, each housing a maximum of 20 residents, so that individuals with memory loss live in small, intimate settings. Over the past two years Oklahoma inspectors visited Iris Memory Care of Edmond five times, and four of those visits ended with no deficiencies cited — including both 2026 visits. Complaints about abuse, falls, dignity, elopement, skin care and staffing were investigated and not sustained, and the May 2025 licensing inspection found the building clean and odor free with enough staff to meet residents' needs. There is one serious exception. In December 2025 the state cited the community after reviewing video of a March 15, 2025 incident: a resident with dementia slid out of bed at 5:47 a.m. and was not helped up until 6:24 a.m. — about 37 minutes. The video showed the aide who came in at 5:48 a.m. did not respond when the resident said 'Can you help,' turned off the bed alarm, was on their phone, provided incontinence care while the resident lay on the floor, and left the room; by 6:18 a.m. the resident was screaming for help and another resident went to find staff. Only one aide was assigned to that building overnight, and the director of nursing confirmed the facility did not call emergency services for lifting assistance. The community retrained all staff on fall response and the state verified in a January 28, 2026 revisit that the deficiency was corrected. The two inspections since then, in January and April 2026, found nothing wrong.
