State inspectors visited Iris Memory Care of Nichols Hills twice in the past two years — a full relicensure survey in November 2025 and a complaint visit in December 2024 — and cited no deficiencies either time. Four separate complaints were investigated on site during those visits, including allegations about staffing levels, about abuse, and about families not being notified of a change in a resident's condition, and none of them resulted in a citation. During the unannounced visits, surveyors wrote that they saw staff assisting residents with activities of daily living and interacting with residents appropriately and respectfully, and they reviewed assessments, care plans, incident reports and nurse notes without finding a problem. Families and residents were interviewed directly about the care being given.
