Iris Memory Care of NW Oklahoma City had one state inspection in the past two years — a full relicensure survey in March 2025 that also investigated two complaints — and it ended with no deficiencies cited. Surveyors specifically looked into allegations of chemical restraint, an unsanitary building, and residents not receiving help with personal care, and none of the three held up: they found behavior medications were prescribed and monitored by specialists and used sparingly, described the community as clean and homelike, and observed residents groomed, dressed appropriately and in clean clothing. Because only one survey falls in this window, this is a smaller record than some communities have.
