Kyrie Assisted Living Center is a faith-informed assisted living community at 801 South Stadium in Ada, Oklahoma. Kyrie Assisted Living had one state inspection in the past two years, and it was a thorough one: over three days in November 2025 the state conducted its relicensure survey while also investigating three separate complaints, covering staffing levels, resident supervision and discharge, and pests and cleanliness. No deficiencies were cited from any of it. Surveyors entered unannounced and wrote that staff answered call lights in a timely manner and were assisting residents, that nursing staff were observed providing care and administering medications, and that residents were interacting with staff and one another. They also interviewed residents directly about their safety and their satisfaction with their care, and reviewed medical records, care plans, incident reports, staffing schedules and grievances.
