What state inspectors found

State inspectors were on site here for three days in March 2025 for a scheduled licensure survey, with two complaint investigations about abuse and food carried out at the same time. They watched staff at meals, in activities, and in residents' rooms and described residents being treated with dignity and respect; they also found the kitchen very clean and the sample meal tasty, hot, and offered with several choices. The visit was not spotless: the state cited the kitchen for opened food items in the refrigerators, freezer, and dry storage that had not been labeled with open and use-by dates, and cited the facility for two of ten sampled residents' annual assessments that were not signed or coordinated by a registered nurse or physician. Both issues were corrected, and an offsite paper revisit in May 2025 found the facility in substantial compliance effective April 30, 2025. This is a good facility to ask questions of directly, since only one inspection falls within the past two years.

  • During the inspection, surveyors watched how staff treated residents at meals, during activities, and in their own rooms, and described residents being treated with dignity and respect. 03-26-2025
  • The surveyor walked into the kitchen unannounced and found the work areas very clean, with all employees following sanitary precautions and no mold on the fresh fruit, vegetables, or bread. 03-26-2025
  • The surveyor was served a sample plate from the kitchen and found the food tasty, served at the right temperature, with several choices available to residents. 03-26-2025
  • Two complaints alleging abuse and mistreatment were investigated on site over three days alongside the licensure survey, and no abuse or mistreatment deficiency appears on the resulting Statement of Deficiencies - the only two citations were for food labeling and for unsigned assessments. 03-26-2025
