Legend at Council Road was inspected five times in the past two years, and four of those visits ended with no citations — including the state's full relicensure inspection in July 2025 and the most recent visit in January 2026, which looked into two abuse complaints and found the community in compliance with regulations on those allegations. One visit, however, was very serious: in October 2025 the state cited the community at its highest severity level (Immediate Jeopardy) for abuse/neglect and for keeping a resident whose psychiatric needs exceeded what assisted living can provide, after that resident attacked a bedbound resident who was hospitalized with severe facial and eye injuries, was discharged on hospice, and died on 09/29/25. Inspectors on that visit also found that several caregivers said they had not received the abuse and mental-health training the community reported giving. Inspectors did verify on site, on 10/22/25, that every step of the emergency (Immediate Jeopardy) removal plan had been completed, and the state accepted the community's written plan of correction on 12/18/25 — but acceptance is a paper step, the two citations themselves remained on the record at an isolated level with potential for more than minimal harm, and OSDH stated it would conduct a revisit to verify all violations were corrected. No revisit report appears in this two-year record. Any family considering this community should read the October 2025 report in full, ask whether the OSDH revisit has taken place and what it found, and ask directly how residents with psychiatric diagnoses are screened, monitored, and moved when their needs change.
