What state inspectors found

Legend at Mingo Road came through all three state inspections in the last two years with no deficiencies cited, including its full relicensure survey in April 2025. During that survey the inspector wrote that residents were clean and well-groomed with no odors, that staff were watchful of residents, and that staffing in the building met state requirements; residents and families said they were receiving the care their contracts described. Four separate complaints — about pests, wandering, and resident safety — were investigated during this period and none resulted in a citation, and inspectors confirmed the exit doors and the memory care courtyard gate are code-locked. This is a clean two-year record, though it is worth knowing that the complaints filed included serious allegations that the state looked into and did not sustain.

  • In September 2025 the state investigated two more complaints — one about preventing elopement and one alleging residents were not free from abuse. Inspectors toured the building, watched residents for any signs of abuse or neglect, asked residents whether they felt safe, and reviewed incident investigations, staffing records and resident council minutes. No deficiencies were cited. 09-04-2025
  • When a complaint alleged supervision was not adequate to prevent a resident from wandering off, the inspector confirmed that every entrance and exit door, and the memory care courtyard gate, open only with a code that staff have. No deficiencies were cited. 06-02-2025
  • All three state visits in the last two years — including the full relicensure survey in April 2025 — ended with no deficiencies cited. 04-22-2025
