State inspectors visited Legend of Edmond twice in the past two years. The July 2025 visit was a complaint investigation into whether a resident had left the building unsupervised - the surveyor observed the memory care unit, staffing levels and residents' behavior, reviewed incident reports and staff time records, and closed the case with no deficiencies cited. The February 2025 licensure survey did find one problem: food in the walk-in refrigerator and dry storage was not properly labeled and dated. The center discarded the unlabeled food, retrained kitchen staff, and the state confirmed the issue was corrected within three days.
