The state cited an immediate jeopardy — its most serious level — in this community’s recent record. These records do not confirm the corrections were verified — worth asking the administrator directly.

What state inspectors found

State inspectors were at this five-bed home on and off between October 1 and October 15, 2025, and the survey that came out of that visit is one of the most serious the state issues. Inspectors cited four separate findings at the "immediate jeopardy" level - the state's most urgent category - including a substantiated report that the administrator smashed a resident's head against a bathroom wall, unlicensed aides giving IV antibiotics through a central line, medications given with no physician orders on file for any resident, and no dietitian, menus, or resident weight records. The facility never provided the state with a plan to remove any of those immediate jeopardy findings, and the administrator was out of the country and unreachable for much of the survey. After reading the entire survey, we could not identify any finding in it that we could honestly present to a family as reassuring, and we would encourage families to read the full survey and speak with the Oklahoma State Department of Health directly before considering this home.

What the state also noted

  • Deficiencies were cited. The survey text states: "A relicensure survey with complaints ... was conducted on 10/01/25 through 10/02/25, 10/06/25 through 10/10/25 and 10/15/25. Deficiencies were cited as a result of the survey."
  • Four deficiencies were at the immediate jeopardy level, the state's most serious level: the dietary consultant rule, the staff qualifications rule, abuse, and the medication administration rule. For each, the survey states a plan of removal was not received and "the deficiency remained at" the immediate jeopardy level.
  • The abuse finding was corroborated by two sources: a former employee who "stated they had witnessed abuse by the administrator," and Resident #4, who "stated staff had smashed their head against a bathroom wall and grabbed their face and shook it back and forth" and confirmed the administrator did it.
  • Additional citations include exceeding the licensed bed count (six residents in a five-bed home, one sleeping on an air mattress in the living room), no privacy divider between unrelated residents of opposite sexes sharing a room, unlabeled and undated food, no food-handler training for any of six staff, no clinical records or physician orders, no abuse policy or abuse training, and no background checks for any of six staff sampled.
  • The five complaint investigation reports do not individually state whether each complaint was substantiated. We counted all five as substantiated because the survey cited deficiencies matching every category of allegation raised (abuse, nutrition and dietitian oversight, administrator availability, nursing supervision, and exceeding licensed capacity).
  • The bundle contains only this one survey, so there is no later inspection in the record showing whether these problems were corrected. No revisit or correction letter appears in the documents reviewed.
