OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings

Maud Residential Care was inspected twice in the past two years. The June 2025 visit included a complaint investigation into whether residents' belongings were returned when they moved out, and the state cited no deficiencies at all from that visit. The March 2026 relicensure survey did find one problem: the home could not show that the State Fire Marshal had completed its required annual inspection during the prior year. The home submitted a correction plan the state accepted, and on May 6, 2026 the state confirmed the deficiency was cleared and the home was back in compliance.

  • The June 2025 inspection, which also covered a complaint investigation, ended with the state citing no deficiencies. 06-03-2025
  • A complaint alleging that residents' belongings were not returned when they left the home was investigated on site by the state, and the investigation resulted in no citation. 06-03-2025
  • For that complaint the surveyor came in unannounced, looked at a sample of eight residents, and gathered evidence through observations and interviews with residents, families and staff. 06-03-2025
  • The one problem found at the March 2026 inspection was fixed and the state confirmed in writing that the home was back in compliance. 03-26-2026
Show all 5 findings
  • The state accepted the home's written plan to keep its safety inspections current. 03-26-2026

What the state also noted

  • One deficiency was cited at the 03-26-2026 relicensure survey under the fire safety rule for missing documentation of an annual State Fire Marshal inspection between 03/2025 and 03/2026; the last inspection on file was 12/03/24. It was corrected and cleared on 05/06/2026, but this is not a spotless two-year record.
  • The clearance of the fire-safety deficiency was done by an offsite/paper revisit on 05/06/26, not an on-site inspection.
  • The June 2025 complaint report's 'Summary of Complaint Investigation' section is blank, and the clean survey form contains no surveyor narrative describing what was observed in resident care, so no conclusions can be drawn about specific care practices from it.
  • Assignment lists this facility as ALF/Memory Care, but the surveys are conducted under Residential Care Home rules (OAC 310:680), which are a different regulatory standard than assisted living.
