Oxford Springs Tulsa passed its March 2025 state relicensure inspection with no deficiencies cited. But a complaint investigation completed in November 2025 found a serious problem: a resident whose care plan called for a two-person transfer was moved by a single aide without a gait belt, fell, and was hospitalized with head trauma and bleeding on and in the brain. The resident returned on comfort measures and died on November 4, 2025. The aide was terminated, the center retrained all direct-care staff and added nurse-observed transfer checks, and the state verified in January 2026 that the deficiency was corrected. This is a recent and severe finding, and families should discuss it and the center's current transfer practices directly with the administrator.
