However, families should know that in May 2025 the state cited this memory care center at the most serious level, immediate jeopardy, after a resident with dementia walked out a front door whose alarm was not working and was found at a nearby grocery store. The center corrected the problem quickly, adding working alarms and key-code locks and retraining staff, and the state lifted the immediate jeopardy within three days and confirmed in August 2025 that all deficiencies were cleared. This is a real safety event worth asking the center directly about, alongside an otherwise clean inspection history.
