State inspectors visited Prairie House three times in the past two years. The most recent visit, in March 2026, was an unannounced investigation of five complaints at once — inadequate staffing and supplies, slow call-light response, failure to assess and respond to a change in a resident's condition, failure to assist with daily care in a timely manner, failure to treat residents with dignity and respect, failure to notify families of changes in condition, unsafe transfers, pests, and pressure sores — and inspectors cited no deficiencies. The two earlier surveys did find problems. In October 2024, after a complaint that a resident had been sexually abused, inspectors found the facility's own investigation of that allegation was incomplete and the administrator said the findings were lost; the state cited the facility for the missing record and made no finding on whether the underlying allegation was true. In May 2025, a licensure survey found that 2 of 5 direct-care staff files reviewed had CPR training papers with no completion date and the administrator could produce no record that the training actually happened, and that none of the 5 files reviewed documented abuse-prevention training within 90 days of hire. The facility submitted correction plans both times, and offsite paper reviews in November 2024 and July 2025 found it back in substantial compliance.
