Prairie Pointe at Stroud had two state inspections in the past two years, and both found problems — so this is not a spotless record. The most reassuring part is what inspectors saw and heard in person: residents told them they felt safe and comfortable, and a June 2025 complaint that raised six serious concerns (medications, grooming, response to a fall, notifying family) resulted in only one citation, for a required incident report the facility never filed with the state after a resident fell and was later hospitalized with a compression fracture. The state noted no actual resident harm was identified, though it also found the deficiencies in both surveys carried the potential for more than minimal harm. One thing to weigh carefully: the March 2025 inspection found the facility had gone without a licensed dietitian since 2021 and was serving from a single menu with no special diets available — ask directly about this if your family member needs a diabetic, low-sodium, or texture-modified diet. The facility corrected everything it was cited for and the health department verified compliance after both inspections.

After the March 2025 inspection found the kitchen had gone without a dietitian, the facility submitted paperwork showing a licensed dietitian went through the kitchen, gave staff an in-service on sanitation, and signed the menus in April 2025. This is documentation the facility supplied as part of its plan of correction, not something a state surveyor observed. 03-27-2025

— Both surveys in the last 24 months resulted in citations; there were no clean surveys in this period.
— The March 2025 licensure inspection cited four problems: no licensed dietitian since 2021, a narcotic dose not documented on the medication record, abuse-and-neglect training not completed within 90 days of hire for 4 of 5 staff files reviewed, and criminal background checks not completed at hire for 3 of 5 staff files reviewed.
— On the dietary citation, the surveyor documented that no special diets were offered at all: the cook said there was only one menu and no special diets, the administrator said there were no specialized menus for special diets, and the menu was not signed by a dietitian. Families of residents needing a medically prescribed diet should ask what has changed since the April 2025 dietitian visit.
— In both survey letters the state characterized the deficiencies as representing "the potential for more than minimal harm," even though no actual harm was identified.
— The June 2025 complaint survey cited the facility for not filing an incident report with the state after a resident fell and was later hospitalized with a compression fracture; the administrator said no report was completed because the resident did not seem hurt at the time.
— The facility's first plan of correction for the incident-reporting citation was found unacceptable and had to be amended before the state accepted it.
