In August 2025 the state ran a relicensure survey at this center over three days and investigated seven complaints at the same time, including allegations of abuse, missing belongings, wound care, involuntary seclusion, and not enough staff. The state cited four things: missing admission and yearly assessment paperwork for several residents, laundry water running about 106-109 degrees instead of the required 160, and meals that did not match the dietitian's written menu. None of the four citations is an abuse or neglect tag — but the state's reports never say whether any individual complaint was substantiated, and two of the citations (laundry, dining) fall in areas that complaints had actually raised, so families should not read this as 'all complaints cleared.' The center submitted an amended correction plan, and an offsite paper revisit on 11/13/2025 found it back in substantial compliance.
