Rainbow Health Care Community and Assisted Living is a family-oriented, faith-influenced provider in the small rural town of Bristow, celebrating 60 years of service as of 2023 and awarded 5-Star Quality Measures for its care. The staff includes physicians, nurses, certified aides, therapists, and support personnel who take a holistic approach: "Staff is dedicated to ensuring the whole body, mind and spirit are cared for." The campus combines assisted living with a broader health care community, including secured memory care units and private and semi-private accommodations.
Residents receive on-site dental and vision services, a beauty shop, and three meals daily with alternatives and snacks, along with housekeeping and laundry. A full activity calendar keeps daily life engaging with "arts and crafts, game time, social times, movie and popcorn, library time, church and music functions," plus "crock pot fun, pizza time and weekly chip and dips." The community also offers a wide range of clinical services including physical, occupational, and speech therapy, restorative and IV therapy, wound care, psychological services, and hospice.

State inspectors visited Rainbow's assisted living three times in the past two years. The most recent licensure inspection, in November 2025, ended with no deficiencies cited, and a January 2025 complaint investigation into infection control also closed with no deficiencies. An earlier licensure survey in October 2024 did cite two problems — a sticky substance on the ice machine door and monthly medication reviews that had not been done since April — but the facility corrected both, and the state confirmed substantial compliance in December 2024.
