Saint Ann Retirement Center is a not-for-profit senior living community in Oklahoma City operated by the Archdiocese of Oklahoma City, welcoming residents of all faiths to "an exceptional retirement living experience." The community emphasizes resort-style living and personalization: "Our goal is to promote residents' individuality and dignity in a friendly, supportive environment," with senior living specialists who develop customized care rather than one-size-fits-all treatments. A nurse is on staff around the clock with 24-hour nurse-aide coverage, and residents benefit from individualized care plans, a visiting physician and podiatrist, and an on-staff chaplain with daily mass in St. Joachim Chapel.
Assisted living services include bathing and dressing assistance, incontinence reminders and assistance, escorts to meals and activities, medication administration and ordering, and dining support such as help opening containers and cutting food, with room-service dining available. Daily life is enriched by restaurant-style meals prepared by a local chef, beauty and barber salons, a game room and grand theatre, a library and computer center with Wi-Fi, wellness and exercise programs, a pet park, and transportation via buses, vans, SUVs, and limousines. The community charges no expensive "buy-in" fee and offers flexible month-to-month agreements.

Care levels, room types and features at Saint Ann Assisted Living. Not sure which of these your loved one actually needs? That’s where I can help — most families are guessing at this part, and guessing wrong is expensive.
Oklahoma inspects every assisted living and memory care community on a regular schedule, plus any time a complaint comes in. Inspectors are state employees, not part of this site — what follows is what they found, in their own words.
Built by Paul Swales, a senior placement advisor, from Oklahoma State Department of Health inspection records — not from reviews. Assisted living and memory care have no federal star rating.
2 inspections in the last two years · 1 with no deficiencies cited · 1 with findings
Saint Ann Assisted Living has a mixed recent record that families should look at closely. A May 2025 inspection that folded in two complaint investigations about resident supervision ended with no findings — surveyors toured the building, watched staff with residents, checked for signs of abuse or neglect, and reviewed health records, incidents and grievances. However, a separate June 2025 complaint investigation did result in a serious citation at the harm level: a resident with dementia missed two doses of a prescribed antipsychotic, and days later walked out the unlocked front door and was found disoriented across a two-lane road. The facility investigated the incident itself, adopted a detailed medication-verification plan, and the state confirmed in August 2025 that the deficiency was cleared.

Every inspection Oklahoma has on record for this community. The rating is based on the last 2 years, so a community is not scored on something it corrected long ago — but the older record is here to read either way.

My help finding the right community costs your family nothing.
You never pay me — the community pays my fee, and only if someone I introduce moves in. And one person calls you back: me, not a call center.

I’m Paul Swales, and this is what I do all day. A senior placement advisor is the person who finds the right community for your family and does the hard part for you. I’ve read the state health department’s inspection record on every community in this metro. I know which ones will genuinely take your loved one’s level of care, and I make the calls — so you’re not doing this alone at eleven at night on your phone.
It works like a buyer’s agent in real estate. Your family never pays me. The community pays my fee, and only if someone I introduce actually moves in. There’s no cost to your family and no obligation to keep using me.
The one catch is the order. They only pay my fee if I introduce you before you contact them yourself. Call them first and you’re a walk-in — I can’t step in as your advocate after that. It costs your family nothing either way. It only decides whether you have someone on your side.
Oklahoma Senior Placement can get you current availability, the real monthly price, and a tour at Saint Ann Assisted Living — at no cost to your family.
