What it’s like

Sand Plum is an independent and assisted senior living center in a beautiful wooded setting in Bixby, and has served the community continuously for about 20 years. The community is dedicated to providing personalized care, with 24/7 in-house medical staff and an emphasis on keeping residents active and engaged through an active social calendar of crafts, games, bus outings, and walking areas.

Daily life is supported by three nutritional meals a day with individualized attention to dietary needs, on-site medication assistance and review, regular housekeeping and laundry, and safe transportation to appointments. One monthly check covers a one- or two-bedroom suite with a patio, a kitchenette in each apartment, weekly maid service, three meals a day, and 24-hour staffing. Community amenities include a game room, library, and fitness room, and the wooded grounds offer walking areas that encourage regular exercise.

Sand Plum Assisted Living Community came through both of its inspections in the last two years with no deficiencies cited. The October 2025 visit was especially thorough — a full relicensure survey combined with five complaint investigations covering abuse, staffing, call light response, pest control, air conditioning, and billing — and inspectors spent four days on site interviewing nine residents and six staff, counting caregivers on duty against the state requirement, and reviewing records, and still found nothing to cite. A year earlier, two more complaints about abuse and staffing were investigated with the same result. A clean record like this is a genuinely good sign, though it is worth knowing that families did file those complaints, so it is reasonable to ask the community how it handles staffing and call light response.

  • The October 2025 inspection was a full relicensure survey combined with five separate complaint investigations, and the state cited no deficiencies at all. 10-23-2025
  • Serious allegations — that residents were not protected from abuse, that staffing was too thin, that call lights went unanswered, and that billing was falsified — were each investigated on site over four days and none resulted in a citation. 10-23-2025
