About

Sand Plum is an independent and assisted senior living center in a beautiful wooded setting in Bixby, and has served the community continuously for about 20 years. The community is dedicated to providing personalized care, with 24/7 in-house medical staff and an emphasis on keeping residents active and engaged through an active social calendar of crafts, games, bus outings, and walking areas.

Daily life is supported by three nutritional meals a day with individualized attention to dietary needs, on-site medication assistance and review, regular housekeeping and laundry, and safe transportation to appointments. OSP RatingOSP GoodHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Gooda clean, solid state inspection record — better than most Oklahoma communities.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

2 inspections in the last two years · 2 with no deficiencies cited

Sand Plum Assisted Living Community came through both of its inspections in the last two years with no deficiencies cited. The October 2025 visit was especially thorough — a full relicensure survey combined with five complaint investigations covering abuse, staffing, call light response, pest control, air conditioning, and billing — and inspectors spent four days on site interviewing nine residents and six staff, counting caregivers on duty against the state requirement, and reviewing records, and still found nothing to cite. A year earlier, two more complaints about abuse and staffing were investigated with the same result. A clean record like this is a genuinely good sign, though it is worth knowing that families did file those complaints, so it is reasonable to ask the community how it handles staffing and call light response.

  • The October 2025 inspection was a full relicensure survey combined with five separate complaint investigations, and the state cited no deficiencies at all. 10-23-2025
  • Serious allegations — that residents were not protected from abuse, that staffing was too thin, that call lights went unanswered, and that billing was falsified — were each investigated on site over four days and none resulted in a citation. 10-23-2025
  • Inspectors physically counted the caregivers on duty and compared it against Oklahoma's required staffing level, reviewed past schedules, and interviewed nine residents and six staff members about whether care needs were being met — and cited nothing. 10-23-2025
  • Complaints about pest control and a broken air conditioner were investigated, including a review of the building's air temperature logs, without a deficiency being cited. 10-23-2025

  • A year earlier, two more complaints — one alleging abuse and one alleging inadequate staffing and slow call light response — were investigated and also closed with no deficiencies cited. 09-25-2024
  • During the 2024 visit, surveyors walked resident rooms and common areas looking at cleanliness and homelike feel, and watched how residents and staff interacted for any sign of fear. 09-25-2024

What the state also noted

  • The surveys do not use the word 'unsubstantiated'; the conclusion that no allegation was sustained comes from the State Form stating 'No deficiencies were cited' for each survey.
  • The Summary of Complaint Investigation sections describe what surveyors looked at and asked about rather than reporting specific positive findings, so the observations above are grounded in the review process plus the deficiency-free outcome, not in explicit praise from the surveyor.
  • Seven complaints were filed against this community across the two-year period, which is a meaningful number even though none resulted in a citation.
  • The text is OCR of scanned pages and contains typographical noise; complaint numbers and dates were read as printed.
