Shawnee Memory Care had one state inspection in the past two years, in October 2025, and it was a big one — a full relicensure survey combined with two complaint investigations alleging abuse and neglect, inadequate supervision, missed meals, and short staffing. Inspectors spent three days interviewing residents and staff and reviewing care plans, staffing schedules, incident reports and grievances, and none of those allegations led to a citation. They described residents as clean, dressed appropriately and free of odors, and watched staff helping residents with daily care and at mealtimes, including carrying trays to residents' rooms. Two things were cited: a dark buildup of grease and debris on the kitchen floor under the cooking equipment, which the dietary manager said had last been cleaned about three weeks earlier, and the fact that the annual fire marshal inspection had not been done in 2025 — the most recent one on file was from March 2024. The state closed both citations effective November 30, 2025, but it did so through an offsite paper review rather than a return visit, and the facility's written correction plan for the fire marshal item was simply to keep calling and leaving voicemails until an inspection could be scheduled. Worth asking the administrator whether a 2025 fire marshal inspection has since been completed, and asking to see it.
